Send a prescription to Pharma and Allied online: a photo, a scan or a PDF of it, with the patient's and the prescriber's details.

What the form asks for

A photo, a scan or a PDF of the prescription: a JPG, PNG, WEBP or PDF file of up to 5 MB.
The patient's name, and a phone number or an email address for them.
The name of the prescriber who wrote it.
Which product it is for, if the shop lists it as prescription-only. This is optional.
